Responsibilities
Matrix Provider - What to Expect:

Conduct Adult/Geriatric Assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation following Matrix guidelines and protocols
Collaborate with healthcare professionals and caregivers in providing services in various settings such as Home Visits, Skilled Facility Visits, and Telehealth Visits

Nurse Practitioner or Physician Assistant - PRN/Part Time - Home Risk Assessments

Type: PRN/Part Time - Flexible-Nurse Practitioner

PAY: $90.00 or $115.00 based on location per completed Assessment

Location: Home, SNF, Telehealth, and other community environments in Buffalo, New York

Hours: Days, Evening, Nights, weekend, Flexible Scheduling

Benefits: PRN/Part Time providers receive 401(k) and limited Voluntary Benefits, Mileage Reimbursement plan

Qualifications
Must Haves:

Master's Degree or equivalent experience with NP licensure
Current RN and NP or PA licensure with the ability to obtain prescriptive authority
Board certified in a Matrix approved specialty
Minimum 1 year experience as a Nurse Practitioner/Physician Assistant
Strong computer skills and EMR software familiarity
Excellent verbal and written communication skills
Positive, outgoing personality, team player, adaptable to change

Travel Requirements:

Valid driver's license, access to a car with adequate insurance coverage
Travel to meet patients at their location
