Description

Responsible for participating in Maternal Infant Health Program (MIHP) patient assessment, intervention, and discharge; and provides care, education, and instruction to patients as well as assists with responsibilities necessary to manager appropriate operations.

This is a full time, benefit eligible position typically working Monday - Friday 8am to 5pm.

Requirements:


  • License to practice Social Work with in the State of Michigan (minimum LLBSW). Valid Michigan driver's license required.
  • One year post degree experience related to Maternal /Child Health, experience in government funded clinics preferred or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job listed above.
  • Bilingual (Spanish) preferred.

Responsibilities:


  • Provide education, care coordination, care plan implementation, and management of Maternal Infant Health Program services to prenatal and postnatal women and infants up to one year. Services are primarily provided in the home.
  • Provide teaching and support on stress and depression, abuse and violence, basic needs, drug and alcohol use, smoking cessation, infant growth and development, parental attachment, and other psychosocial issues as needed.
  • Make referrals to community resources to address the needs of pregnant and parenting patients.
  • Completes MIHP risk screening assessment on new patients, progress notes on all visits, and discharge summaries upon completion of MIHP services within time requirements.
  • Participate in interdisciplinary planning with nurses, dieticians, medical providers, and community health workers for meeting patient needs and health service objectives.
  • Participate in ongoing quality assurance activities, including chart auditing and caseload reporting.
