    Residency Special Features: Northwest Medical homes (formerly Springfield Family Physicians) represents the largest independent primary care organization in Lane County. The three clinics situated in the Eugene/Springfield zone are supported by 1-2 clinical pharmacists for every 5-10 providers. Acknowledged as a Tier 5 Patient-Centered Primary Care Home (PCPCH), showcasing a dedication to inventive, team-based care. This PGY2 residency will emphasize delivering the most advanced ambulatory care model. The pharmacy department collaborates with behavioral health and medical providers in a co-consultation model, and autonomously under collaborative drug therapy management agreements (CDTM) for various disease states. Other obligatory learning opportunities cover administration/leadership, teaching, business/new service development, community health at the local Federally Qualified Health Center (FQHC), population health, and a continual ambulatory care staffing experience in all three clinics. Residents will have the chance to pursue a teaching certificate facilitated by the two local colleges of pharmacy, besides mentoring a PGY1 resident or a pharmacy student. Projects encompass teaching during staff meetings, a perpetual research project focusing on the establishment or evaluation of an ambulatory care service, and upgrading or creating a CDTM. After completion, our resident will possess the knowledge and competencies to establish and substantiate their ambulatory care role in diverse environments. **The commencement date of the residency can be adjusted based on the resident's requirements

    Special Requirements for Acceptance: Pharm.D. degree; Successful completion of PGY1 residency; Curriculum vitae; Three recommendation letters; Finished application; Post successful results of the Oregon pharmacist law exam before November 1st; Passing a background examination; Clearing a drug test; BLS certification; and ability to carry out vaccinations.
