Job Summary and Qualifications:

What you will do in this role:

You will assist and coordinate intake and pre-admission screening processes. You will document evidence of rehab admission criteria within established timeframe.
You will perform onsite and offsite clinical evaluations to assist Rehab Medical Director in establishing patient eligibility for admission.
You will provide information to patient/family/caregiver related to inpatient rehab and program features. You will communicate expectations and requirements for participation in the rehab program and conduct or coordinate tours of the rehab unit.
You will obtain information relating to patient resources and healthcare insurance benefits.
You will demonstrate clinical knowledge of rehab admission criteria to appropriately identify and qualify rehab patients.

What qualifications you will need:

Associate Degree
Graduatge of an accredited school in a clinical specialty
Current state licensure in clinical practice area or eligible for state license transfer
Speech Therapist, or Licensed Physical Therapy, or Occupational Therapy (OT), or Registered Nurse, or Registered Respiratory Therapist Eligible (RRT)
Minimum 2 years recent clinical care experience
