    Summary:


    The Transplant Coordinator RN 2 is responsible for coordination of the evaluation and preparation of patients for organ transplantation and long-term management of patients after transplant.

    The Transplant Coordinator is responsible for coordinating care across multiple areas in order to provide continuity of patient care across settings.

    This position is also responsible for performing initial assessment of transplant evaluated patients and providing follow-up post-transplant care, which could be performed in hospital or outpatient clinics.

    • Ensures the potential transplant recipient has met the solid organ transplant criteria.
    • Conducts pre-transplant interviews with prospective transplant patients and families. Interprets eligibility criteria for patients, families, physicians, and outside agencies.
    • Prepares and analyzes pre-transplant clinical data and collaborates with other medical personnel in preparation of patients and families for transplantation; obtain medical and social histories.
    • Works cooperatively with social work, psychiatry, and patient financial liaisons to assess patient's psychosocial candidacy including financial concerns.
    • Informs patients of the rules and regulation of transplant allocation developed by United Network for Organ Sharing (UNOS).
    • Composes written transplant summaries, clinic visit and summaries and letters.
    • Autonomously presents the patients' candidacy to the multi-disciplinary team, prepares the patients information for candidacy to medical and surgical team discussion and conscientious for listing patient on the national organ transplant waitlist to receive organ offers.
    • Performs on call responsibilities for those patients' awaiting transplantation.
    • Arranges for and communicates about patient admission and history to appropriate team members.
    • Shares on-call and weekend responsibilities as assigned, in order to assure Transplant Coordinator coverage after hours and on weekends.
    • Reviews and interprets the organ match run to identify the appropriate Rush transplant candidates.
    • Presents critical aspects of the organ offers to the Transplant Surgeon focusing on donor history, hospital course, medications and major treatment interventions, laboratory review including serology, and blood type. Requires clarifying communication between the offering Organ Procurement Organization (OPO) and the OPTN to obtain complete clinical, psychosocial, infectious disease, laboratory, and logistical information on the organ donor and specific organ offer. Provides advanced patient selection assistance to the transplant surgeon and assists in reviewing, screening, selecting, and recommending transplant candidates based on organ specific criteria.
    • Coordinates the transplant event process which includes patient hospital admission set up, clinical team notification to include surgery, OR, ER, AICU Team, security, etc. Coordinates start times of organ recovery and organ transplant. Coordinates the recovery of the organ to include transportation logistics (ground and air), timing, and retrieval.
    • Conducts post-discharge planning and education for transplant patients.
    • Compiles and reviews post-discharge data for clinical management of transplant patients.
    • Provides telephone consultation and follow-up to support patients and their families.
    • Maintains patient status in EMR and UNOS based on guidelines established by UNOS and CMS.
    • Assesses, plans, and evaluates interventions for patients in the clinic or remotely.
    • Collaborates in the development of protocols and guidelines for patient management.
    • Develops, implements, and evaluates comprehensive patient education programs.
    • Participates in the development and execution of quality assurance programs and projects.
    • Ensures that the Rush Transplant Program remains in compliance with the National Organ Procurement and Transplant Network (OPTN), established by the Federal Government.
    • May serve as a peer-to-peer coach for onboarding new team members and transplant coordinators.
    • Supports planning of and participates in clinical research projects.
    • Forms the nursing process (assessment, care planning, intervention, evaluation) independently and with high level of expertise within the limits of licensure and scope of practice for patients with various states of end stage organ disease and transplantation.
    • Provides population management for case load of patients.
    • Follows Rush University Medical Center and Solid Organ Transplant Program policies, procedures, techniques, and guidelines.
    • Creates and maintains appropriate documentation for transplant candidacy and post-transplant follow-up care in accordance with CMS and UNOS requirements, in required timelines.
    • Provides required education across the continuum of the transplant process appropriate to the phase of care to transplant candidates, families, and significant others in accordance with CMS and UNOS requirements.
    • Reports listing of patients on the waitlist, changes of patient status, time of transplant and follow-up via TIEDI in required timeframes.
    • Ensures completion of all transplant evaluation requirements (diagnostic, laboratory and consultative) to be presented at weekly Selection Conferences.
    • Prepares, presents, and effectively communicates patient evaluation findings to a multidisciplinary team of physicians, surgeons, and ancillary services at weekly Selection Meetings to determine transplant candidacy in accordance with the Solid Organ Transplant Program policies and procedures.
    • Participates in survey-readiness and understands and complies with regulatory requirements of planning, regulatory, inspecting agencies, and solid organ transplant policies (Medicare Conditions of Participation; Department of Health Services; Joint Commission; United Network for Organ Sharing).
    • Completion of TIEDI form to meet CMS requirements of greater than or equal to 95% completion within specified time frame across the continuum of care.
    • Demonstrates ability to dialogue with surveyors effectively. Actively participates in CMS Transplant focused and UNOS surveys, during interviews and patient tracer audits providing evidence to show regulatory requirements are being meet.
    • Other duties as assigned.

    Required Job Qualifications:

    • Current State of Illinois Registered Nurse licensure required.
    • Current BLS/CPR certification required.
    • Two years of nursing experience required.
    • Earned Nursing Degree at the Baccalaureate level or higher for those hired after January 1, 2014. All transfers to Rush University Medical Center or Rush University Medical Group from other Rush entities must have an earned nursing degree at the baccalaureate level or higher prior to the transfer.
    • Analytical ability.
    • Ability to use discretion in decision-making within department policies and principles.
    • Ability to prioritize clinical issues and case manage a large number of patients in various stages of the transplant process.
    • Ability to demonstrate competence and management in the care of people with renal and hepatic failure, diabetes, cardiopulmonary issues, hypertension, hyperlipidemia, and other disorders related to transplantation.
    • Communication skills are required in order to lead and teach.
    • Ability to interact effectively with others in difficult situations.
    • Ability to participate in an on-call rotation.
    • The ability to check documents for errors, use a keyboard to enter or retrieve data, and closely examine specimens, images or reports.
    • The ability to apply knowledge of the United Network for Organ Sharing (UNOS)/Organ Procurement and Transplantation Network (OPTN), the Centers for Medicare and Medicaid Services (CMS) Condition of Participation (CoP) for Transplant Programs, and the Joint Commission governing organ donation and transplantation in the US is required to ensure hospital and transplant program adherence to all regulations.

    Preferred Job Qualifications:

    • National certification preferred.
    • Two years of medical/surgical experience preferred.
    • One year of organ failure/transplant experience preferred.
    • Certified Clinical Transplant Nurse preferred.

    Physical Demands:

    • Ability to walk throughout the Medical Center and to be standing or walking most of the designated shift.
    • Lifting or carrying objects 35-40lbs. and lifting and positioning patients, some of whom may exceed 300 lbs.
    • Work conditions include performing procedures where carelessness could result in injury or illness and/or contact with potentially infectious materials and/or strong chemical agents.
