- Outreach and engagement to formally enroll referred individuals into the care management program.
- Conducts the initial Child and Adolescent Needs and Strengths (CANS) assessment to evaluate the needs and strengths of the child and family.
- Develops the initial care plan driven by the CANS assessment and updates the care plan based on the changing needs of enrollees. Monitors the care plan to assure adherence to the principles of health home and supports health home policies and procedures.
- Coordinates and facilitates ongoing Care Family Team meetings that include the child, family, natural supports and any/all network providers assisting in the delivery of the Care Plan
- Assures that necessary supports are in place to assist the child and family in carrying out the care plan inclusive of both community service providers and natural supports as identified by the child and family such as peer and family contacts.
- Ensures that all of the initial linkages are established and maintained and advocates for additional services and linkages as appropriate.
- Collaborates with all services providers establishing both a team communication plan and a crisis plan for the child and family.
- Provides ongoing monitoring of Care Plan goals, maintains current care management documentation and information regarding care management activities within all required health information technology systems used by the Health Home.
- Completes required Care management documentation.
- Performs other duties as needed.
- A Bachelors of Arts or Science Degree in psychology, sociology, human services or related field with two years of relevant experience, or
- A Licensed Registered Nurse with two years of relevant experience, or
- A Master of Arts or Science Degree with one year of relevant experience.
- 3 weeks of paid time off in your first year, 4 weeks in your second year.
- 48 hours of Wellness time annually (4 hours accrue per month) that can be used any time you feel you need some time for yourself.
- 96 hours of Sick time annually (accruing 8 hours per month)

Job DescriptionCayuga Counseling Services is seeking a Care Manager in Cayuga County to serve as the primary point of contact with the enrolled child and their family to provide ongoing advocacy and support to ensure the care coordination network of providers delivers comprehensive, integrated physical and behavioral health care that recognizes their unique needs, is family-and-youth driven, and supports a system of care that builds upon the strengths of the child and family/caregiver.

(Relevant experience includes proficiency in the human services or medical field in the area of mental health, substance abuse, public health, early childhood or established proficiency on an individual applicant basis).
Successful applicants must possess: Ability to organize and prioritize duties to meet job demands, make independent decisions, successfully engage children and their families; Basic Computer Skills (Windows, Outlook, Adobe Acrobat, Word, Excel); Travel is required-Must have a valid NYS Driver's License; Knowledge of the child serving delivery systems preferred.
