Reporting to the Manager of Quality and Population Health, the Process Improvement Specialist is responsible for managing the electronic medical record (EMR) database, compiling and presenting reports, and analyzing data related to all aspects of the Patient-Centered Medical Home model to enhance efficiency and promote transformation across all clinic processes and workflows.


Process Improvement

  • Facilitate and support ongoing performance improvement activities and initiatives to accomplish measurable project outcomes.
  • Uses performance improvement methodology, tools, and techniques in organizational operations to accomplish objectives and outcomes that improve the effectiveness and quality of healthcare.
  • Works with organization leaders and managers to plan, design, implement, and achieve the desired outcomes of key improvement projects.
  • Provides support and consultation to quality and operational improvement teams related to transformation and performance improvement initiatives.
  • Provides education on performance improvement methodologies to various Hamdard teams and staff.
  • Utilize the Patient-Centered Medical Home (PCMH) model to analyze outcomes and work with staff to ensure compliance across all operational and clinical measures.
  • Generate, monitor, and analyze data related to performance improvement for all staff.
  • Work with organization leaders and managers to plan, design, implement, and achieve the desired outcomes of key improvement projects.

Data Analytics

  • Build, run, and monitor reports in the EMR. These reports provide data for grant reporting, clinical workflows and effectiveness, clinical quality improvement dashboards, phone utilization metrics and access to care, and process improvement coaching.
  • Analyze and monitor reporting tools external to the organization, including but not limited to the phone system, lab vendor, and patient experience.
  • Work collaboratively with organizational leadership to design metrics and tools for measuring process and team outcomes.
  • Perform security and clinical audits and investigations using reporting functions within the EMR.

EMR Subject Matter Expert and Administrator

  • Provide technical support for the EMR and telehealth platforms. Coordinate with HR and managers on all account management during staff onboarding and offboarding.
