A DAY IN THE LIFE OF A HOSPICE LICENSED PRACTICAL NURSE
As a Licensed Practical Nurse, you make it your mission to provide the best possible care for your patients and their families. Working under the direction of the RN case manager, you provide services according to the plan of care. You continually assess the needs of your patients and report any signs of abuse or changes in their health as well as make recommendations for changes to the plan of care. To keep the family and patient informed, you counsel them regarding advance directive issues such as living wills and healthcare surrogate forms. You also maintain up-to-date and accurate patient records to ensure continuity of care.
In addition to working with the patient and their family, you also establish and maintain positive relationships with vendors served by our agency. You enjoy the opportunities you have to participate in our Total Quality Improvement program, training activities, and community programs that all promote professional growth. As a team player, you participate in the on-call rotation, occasionally assist with the orientation of new nurses, and assist with discharge planning. You feel great about the positive impact you have in the lives of your patients and their families
QUALIFICATIONS
Experience with acute care nursing is preferred but not required. Do you have excellent interpersonal and communication skills? Can you work well as part of a team? If yes, you might just be perfect for this position
