The Ridge Post Acute with 54 beds located in the east of San Jose, California. We understand the individuality of each person, and our healthcare staff is dedicated to achieving the maximum level of a resident's potential while providing a compassionate environment.

We are looking for a committed, caring, and reliable
Director of Social Service to join our in-house rehab team for our Skilled Nursing Facility

RESPONSIBILITIES:


  • Develop and implement strategic plans and initiatives to improve the delivery of social work services
  • Oversee the daytoday operations of the social service department, including budgeting and resource allocation
  • Recruit, train, and supervise a team of social service professionals, ensuring highquality care and service delivery
  • Collaborate with interdisciplinary teams to develop and monitor care plans for clients, addressing their social and emotional needs
  • Conduct assessments and evaluations to identify clients' strengths, challenges, and goals
  • Provide direct social work services to clients, including counseling, crisis intervention, and advocacy
  • Foster and maintain effective relationships with community organizations and resources to enhance social support for clients

REQUIREMENTS:


  • Bachelor's degree in social work or related field; master's degree preferred
  • Minimum of 1 year of experience in social work, preferably in a healthcare setting
  • Strong knowledge and understanding of social work principles and practices
  • Proven experience in program development and evaluation
  • Excellent leadership and team management skills
  • Exceptional communication and interpersonal abilities
  • Ability to multitask, prioritize work, and meet deadlines in a fastpaced environment

EXPERIENCE:


  • With Skilled Nursing or Long-Term Care experience is preferred
  • Experienced Director of Social Service
  • Similar leadership roles in a healthcare setting are welcome
