Description

We are currently seeking a Chaplain for our hospice.

( Spanish speaking preferred)This is a part-time/Per Diem position and their primary function is to provide a compassionate spiritual care and bereavement plan to patients and caregivers and perform spiritual assessments and support.

Responsibilities:

- ‪‪ Assess hospice patient/family to identify spiritual needs and develop an individualized spiritual plan of care in accordance with patient's and family's acceptance of these services and in a manner that is consistent to their spiritual and religoius beliefs and desires.

• Collaborate with team members and local clergy as needed
• Participates in IDG to establish and maintain the appropriate spiritual plan for hospice patients
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparations of advance directives, funeral planning and transfer of responsibilities relative to legal and fiscal issues.
• Work with other staff, resolving spiritual/ pastoral/ethical issues. Be available for bereavement or counseling needs.

Qualifications:

• Certificate or Theological studies highly preferred.
• Ordained, credentialed, or commissioned according to the practices of a recognized religious sect, or a division of a recognized religious sect.
• Working knowledge of community resources, along with strong interpersonal, ethical, and pastoral skills
• Ability to work with an Interdisciplinary group in the formulation of a Spiritual Plan of Care.
