    • Friday with some weekends 8 hour shifts at our 180 bed full service acute care facility.
    We offer a family friendly work environment, perfect for a new grad looking to start their career or an experienced nurse looking to make a change As a Registered Nurse Care Manager you will be responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. You will facilitate the progression and transition of care using established criteria in conjunction with the multidisciplinary team.

    You will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

    Crestwood Medical Center is located in Huntsville, AL which offers a variety of attractions, such as education, arts, nature or just pure entertainment.

    You can relax and take a hike outdoors or visit one of the numerous spas. If shopping is more your speed, there just isnt a better destination in North Alabama. When the sun sets, Huntsvilles nightlife has the charm of a big city without any of the hassle.

    We have been recognized as an Accredited Chest Pain Center with Primary PCI and Resuscitation by American College of Cardiology, a Primary Stroke Center from The Joint Commission and American Heart Association, a Blue Distinction Center for Bariatric Surgery, Maternity Care and Spine Surgery, the American Heart Association's Get With The Guidelines for Heart Failure (Gold Plus) and AFib (Gold), a Certified Treatment Center of Excellence from the ALS Association, and other accreditations and recognition for Imaging, Endoscopy, Breast Care, and Elderly Care.

    Requirements Required Education:
    Associate Degree in Nursing or

    Nursing Diploma Preferred Education:
    Bachelors Degree in
    Nursing Required Experience: 2 years experience in Care Management or 3 years experience in clinical nursing

    Preferred Experience:
