***: Reporting to the Director of Health Services, the Create to Activate (C2A) Project Coordinator facilitates and coordinates the C2A Project at the Huckleberry Youth Health Center in San Francisco. The C2A program is a part of a three year state grant to provide Transitional Aged Youth (TAY) between the ages of 16-24, with a peer-to-peer led space to discuss, collaborate and create programming on the impacts substance use has played in their own communities and the greater Bay Area with an emphasis toward the impacts of the fentanyl epidemic. The project's goal is to provide a client-centered space for youth to use their own voices through the creation of zines and other forms of media, leading into a culminating event presented to their larger community. The C2A program is embedded into the Health Center team that includes case managers, therapists, community health educators and medical staff from the Department of Public Health.


This is a term-limited position, which runs from the hire date until December 2026, with the possibility to continue should funding become available.


MAIN PROJECT ACTIVITIES INCLUDE:


  • Building and maintaining relationships with communitybased partners, schools, and youth through outreach activities.
  • Take a leading role in program planning and curriculum development and recruitment of youth, with assistance from the Director of Health Services and other team members, as assigned.
  • Mentoring of youth on a oneonone basis including facilitation of training in collaboration with other team members, communitybased partners, and the Department of Public Health to enhance preliminary understanding of the scope of the project.
  • Facilitating focus groups with Transitional Aged Youth (TAY) before programming begins and after in order to gather qualitative data to support the grant.
  • Take a leading role in ensuring internal collaboration with other teams at the Health Center Site and across other programs within Huckleberry.

ESSENTIAL FUNCTIONS:


  • Conduct four, eight week cohorts of youth in training, building and creation of Zine making/other forms of media with assistance from the Director of Health Services and other team members as assigned.
  • Conduct oneonone sessions with youth in the cohort to develop individual projects, individual growth plans, as well as supporting their needs within the group and connecting them to any needed resources provided by Huckleberry Youth Programs or other communitybased partners.
  • Recruitment activities include interviewing and selecting members of the four cohorts with assistance from the Director of Health Services and other team members, as assigned.
  • Assist with developing components to the Create to Activate program including but not limited to: development and facilitation of four training sessions for each cohort, connecting with community members as guest speakers, recruiting and onboarding youth for programming, developing curriculum and outline for each cohort, facilitating peertopeer learning in the cohort, and facilitating the planning and execution of four culminating events including two larger events for the implementation of youth created projects such as a youthled panel.
  • Plan and conduct community outreach activities and events to promote services available at the Health Center and as part of cohort recruitment.
  • Participate in intersite and interagency wide collaboration

This can include:

training of other Huckleberry staff, coordination of services and referrals, cross program planning, and events such as the HYHC Block Party, AIDS Walk, etc.


  • Attend Health Center site meetings, team meetings, supervision meetings, agency meetings and required trainings
  • Collaborate with the team on all efforts to achieve grant deliverables, this includes ensuring data attendance is up to date, and completing monthly staff reports.
  • Maintain uptodate knowledge in the fields of sexual and reproductive health, reproductive justice, positive youth development, clientcentered care and harm reduction
  • Assist with front desk duties as assigned such as client registration and navigation of clinic flow, answering and redirecting phone calls, and other duties assigned by the Program Coordinator or Director of Health Services.
  • Other tasks and responsibilities as assigned.

COMPETENCIES:


  • Ability to take initiative and work proactively
  • Passion for connecting with and engaging young people in a 1:1 setting and group setting
  • Experience training, teaching, or facilitating groups with youth
  • Experience collaborating with staff partners at school sites or other community agencies
  • Proven history of excellent verbal communication and presentation skills
  • Proven history of successful group facilitation
  • Commitment to adolescent health, equitable healthcare access, reproductive justice, and an understanding of substance use and harm reduction
