Memory Care Assistant


Responsibilities:


  • Conduct comprehensive assessments of residents' cognitive and functional abilities.
  • Develop and implement individualized care plans based on assessment findings.
  • Provide training and support to staff members on memory care best practices.
  • Design and oversee engaging and therapeutic memory care programs and activities.
  • Collaborate with healthcare professionals for comprehensive resident care.
  • Communicate regularly with residents' families, offering support and updates on care plans.
  • Manage medication administration and compliance with protocols.
  • Handle crisis situations effectively, implementing appropriate interventions.

Qualifications:


  • Bachelor's or Master's degree in Nursing, Social Work, Gerontology, or a related field.
  • Previous experience in a memory care or dementia care role.
  • Certification as a Certified Dementia Practitioner (CDP) or equivalent is preferred.
  • Strong interpersonal and communication skills.
  • Compassion, patience, and a commitment to providing exceptional care.
