    Description


    This position is responsible for certifying housing clients for RW-Part B service eligibility every 6 month at AFC as well as at subcontractor agencies based in Cook County as well as the Collar Counties.

    Priorities for AFC Housing include ensuring that clients are stably housed and engaged in quality medical care. The Certification Specialist will engage with clients, direct service staff, and medical providers to obtain needed data. Assistance will also be provided to Housing Directors and Coordinators in identifying and rectifying data quality issues.
    The salary for this role is $50,000 annually.

    ESSENTIAL FUNCTIONS AND RESPONSIBILITIESClient Outreach and AssessmentConduct face-to-face and over the phone eligibility assessments and reassessments every six monthsCollect documents from the client or service providers at subcontracted agencies for the certifications/reassessmentsCreate service plans that match the needs of the client identified in the assessmentsEnsure medical provider info is documented in database and appropriate releases of information is on fileOutreach to medical providers to obtain client medical data, i.e.

    viral load, last medical appointment dateData Entry and Quality AssuranceEnter all assessment, billing, and other required data elements into Provide Enterprise databaseAdhere to date entry timelines as outlined by IDPHInput quarterly case notes for all IDPH clientsProvide database technical assistance to case managers and housing technicians as neededParticipate in reconciliation of payments between Provide and Financial EdgeUtilize and interpret Tableau reports to ensure quality of data enteredLiaise with finance, data services, and CARE team to ensure accurate data entry by self and contracted partner agenciesOtherAssist with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, World of Chocolate and othersProtect organization's value and manage risk by keeping information confidentialPerform other duties as assignedThe list of essential functions, as outlined herein, is intended to be representative of the tasks performed within this classification.

    SUPERVISORY RESPONSIBILITIESNoneENTRY REQUIREMENTS (EXPERIENCE AND EDUCATION)Minimum QualificationsThree (3) years of direct service programs administration experience, orThree (3) in social services, health care, or related fieldPreferred QualificationsBachelor's degree in a human services fieldOne (1) year of direct service experienceOne (1) year of programs administration experience in social services, health care, or related field.

    KNOWLEDGE, SKILLS, AND ABILITIESThe ability to use computer and web-based systems (e.g., PC-based tools, Microsoft applications, Web-based applications)The ability to provide efficient, quality service to both internal and external customersThe ability and willingness to uphold ethical standards and comply with all relevant laws and company policies and proceduresThe ability to develop and maintain professional, trusting, and positive working relationships with mangers, supervisors, staff, co-workers, and vendorsREQUIRED CERTIFICATES, LICENSES, REGISTRATIONSNone.

    PHYSICAL DEMAN DS AND WORK ENVIRONMENTThe physical demands are representative of those found in a general office environment. The work environment is representative of that found in a general office environment. Tasks involve moderate physical activity requiring some agility and dexterity, including periods of walking, standing, or bending.

    Tasks may involve some lifting, carrying, pushing and/or pulling of objects and materials of moderate weight (up to 20 pounds.) Tasks may also involve extended periods of time at a keyboard or workstation and on the telephone.
