Description

This position is responsible for certifying housing clients for RW-Part B service eligibility every 6 month at AFC as well as at subcontractor agencies based in Cook County as well as the Collar Counties.

Priorities for AFC Housing include ensuring that clients are stably housed and engaged in quality medical care. The Certification Specialist will engage with clients, direct service staff, and medical providers to obtain needed data. Assistance will also be provided to Housing Directors and Coordinators in identifying and rectifying data quality issues.

The salary for this role is $50,000 annually.


ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Client Outreach and Assessment

  • Conduct facetoface and over the phone eligibility assessments and reassessments every six months
  • Collect documents from the client or service providers at subcontracted agencies for the certifications/reassessments
  • Create service plans that match the needs of the client identified in the assessments
  • Ensure medical provider info is documented in database and appropriate releases of information is on file
  • Outreach to medical providers to obtain client medical data, i.e. viral load, last medical appointment date

Data Entry and Quality Assurance

  • Enter all assessment, billing, and other required data elements into Provide Enterprise database
  • Adhere to date entry timelines as outlined by IDPH
  • Input quarterly case notes for all IDPH clients
  • Provide database technical assistance to case managers and housing technicians as needed
  • Participate in reconciliation of payments between Provide and Financial Edge
  • Utilize and interpret Tableau reports to ensure quality of data entered
  • Liaise with finance, data services, and CARE team to ensure accurate data entry by self and contracted partner agencies

Other

  • Assist with agencywide activities as directed, including Annual Meeting, AIDS Run & Walk, World of Chocolate and others
  • Protect organization's value and manage risk by keeping information confidential
  • Perform other duties as assigned

ENTRY REQUIREMENTS (EXPERIENCE AND EDUCATION)

  • Minimum Qualifications_
Three (3) years of direct service programs administration experience, or

Three (3) in social services, health care, or related field

  • Preferred Qualifications_
Bachelor's degree in a human services field

One (1) year of direct service experience

One (1) year of programs administration experience in social services, health care, or related field.

The ability to provide efficient, quality service to both internal and external customers

PHYSICAL DEMANDS AND WORK ENVIRONMENT**The physical demands are representative of those found in a general office environment. The work environment is representative of that found in a general office environment. Tasks involve moderate physical activity requiring some agility and dexterity, including periods of walking, standing, or bending. Tasks may involve some lifting, carrying, pushing and/or pulling of objects and materials of moderate weight (up to 20 pounds.) Tasks may also involve extended periods of time at a keyboard or workstation and on the telephone.
