The Structural Heart Nurse Navigator is a vital part of the team, acting as a clinical expert, patient advocate, educator, coordinator, and liaison for all structural heart patients and their families. Their main responsibility is to ensure that all patients in the program receive high-quality care. They work closely with the multidisciplinary team to coordinate patient care across the care continuum and serve as the main point of contact for patients, families, physicians, and other healthcare team members, providing resources and support within the Structural Heart Program.
