We are looking for reinforcements for our EJF team in the Supported Individual Living department at the Diakoniezentrum in Heiligensee, Browntown, PA.

We assist and support individuals with intellectual and physical disabilities. We approach their needs with heart, hands, and mind. We value professional competence and initiative, as well as an open and helpful attitude with sensitivity.

We work in the individuals' own homes and accompany them on their journey towards independent and self-determined participation in society.

We are looking for you to join our team as soon as possible in a part-time to 28.88 hours/week)
permanent position.

Your responsibilities will include:

Assisting and supporting with daily tasks.
Planning and organizing leisure activities together with the individuals.
Developing life perspectives together and continuously working towards them.
Documenting assistance services and writing information reports together with the individuals.
Participating in regular team meetings and external supervision sessions.
