Description

CWTC is looking for a part time Community Housing Nurse to provide care for our consumers in our CILAs (Community Integrated Living Arrangement) and CLFs (Community Living Facilities).

Nurses will be participating as a member of the team will be assisting individuals to be as independent as they possibly can.


ESSENTIAL DUTIES:


  • Complete the IDHS RN Nurse Trainer Course if not already certified.
  • Train Direct Care Staff and Case Managers on Medication Administration according to Rule 116
  • Complete initial move-in assessments as well as annual assessments of residents.
  • Compete progress/case/nursing notes after resident contact.
  • Review and approve medication changes and provide necessary medication information to direct care staff.
  • Review, sign, and file ancillary forms noting important information necessary to continuity of care.
  • Oversee medication errors and implement corrective action in coordination with the Safety Director.
  • Complete additional staff training and reviews as required and/or as needed.
  • Review and sign off on all resident's annual program plans.
  • Serve as a liaison between the hospital and residential facility for residents who are receiving in-patient services and evaluate the resident before discharge to ensure appropriate placement.
  • Serve as an active member of the Human Rights Committee by attending quarterly meetings.
  • Maintain accurate records in the electronic MAR database as well as in resident medical charts.
  • Develop a relationship with the agency's contracted pharmacy to utilize electronic MAR to provide medications in the safest manner possible.
  • Reads and reviews physician order sheets to check for accuracy in preparation for physician signatures.
  • Track appointment dates and communicate needed appointments with house managers to confirm all appointments are taking place in a timely manner.

OTHER DUTIES:


  • Coordinate appropriate staff and resident training with the Safety Director.
  • Perform other tasks, not inconsistent with qualifications or regular duties as assigned.
  • Perform as a member of the code blue team when inside CWTC's main building.

WORKING CONDITIONS:

Physical conditions include sitting for long periods, good hand dexterity, good hearing and vision, and the ability to communicate clearly.

Environmental conditions include working in an office environment and spending long periods in front of a computer monitor in the facilities or at an offsite location.

Interpersonal conditions include the ability to communicate effectively with adults with disabilities.

RELATED CONDITION:

All employees are responsible for reporting any information, however acquired, about possible abuse or neglect of consumers to the Safety Director or the Executive Director.


SAFETY AND ACCIDENT PREVENTION:
Employees must follow all safety policies and use necessary protective equipment when applicable. Employees are responsible for reporting safety hazards and/or accidents to their supervisors immediately.
