- Perform Adult/Geriatric Assessments, medical history reviews, diagnosis, treatment, health education, referrals, and clear documentation following Matrix guidelines
- Collaborate with healthcare professionals and key stakeholders to ensure comprehensive care
- Work in various settings including home visits, skilled facilities, and telehealth appointments
- Master's Degree or relevant experience with NP licensure
- Current RN and NP licensure with prescription authority or eligibility for prescriptive authority
- Board certification in a Matrix-approved specialty
- BLS, ACLS, or CPR certification
- Minimum 1 year of NP experience
- Proficiency in computer skills and medical record software
- Excellent communication skills
- Adaptability to change
- Ability to travel with a valid driver's license and reliable transportation
- Travel required to reach patients' residences

Matrix providers conduct thorough health assessments during visits, focusing on a member's health, medical history, medication adherence, social environment, and other potential risks. This approach provides valuable insights into the member's overall well-being, leading to improved quality of care and the ability to address multiple care gaps in a single visit.

Matrix Provider – What You'll Do:
Nurse Practitioner - Part Time/Per Diem - Home Risk Assessments
Type: Part Time/Per Diem - Flexible-Nurse Practitioner
Compensation: $95.00 or $100.00 based on location per assessment
Location: Home, SNF, Telehealth, and other community settings in Granbury, Texas
Hours: Flexible scheduling, including days, evenings, nights, and weekends
Benefits: Part Time/Per Diem roles offer limited benefits, including 401(k) and voluntary benefits
Mileage reimbursement plan available
