Greater Family Health is a multi-site federally qualified health center providing comprehensive medical, dental, and behavioral health services within the Chicagoland area.
Our mission is to provide quality, affordable health care for all including those without the ability to pay. We currently have health centers in DeKalb, Elgin, Franklin Park, Hanover Park, McHenry, Palatine, Streamwood, Sycamore, and Wheeling.Your Goal:
This role is responsible for monitoring, assessing, evaluating, educating, and providing education and referrals to facilitate the coordination of medical health, oral health, and social services for low-income families.
Key Responsibilities:
- Provide daily management and oversight of Family Health Services support staff and operations.
- Deliver all forms of leadership, supervision, direction, training, and role modeling to all subordinate staff. Schedules staff appropriately, according to clinic needs.
- Coordinate with Human Resources department. in interviewing, hiring, and training new employees.
- Assist in development, interpretation and implementation of new policies and procedures.
- Ensure staff competency in their performance of family case management tasks and procedures.
- Accurately documents client activity in Cornerstone.
- Monitor and assess client/family needs.
- Provide Case Management in accordance with the standards of the Illinois Department of Human Services Family Case Management Program (IDHS FCM).
- Ensure compliance with Greater Family Health Policies and Procedures, Joint Commission Standards and all other applicable laws and regulations.
- Perform other duties as assigned.
Qualifications:
- Preference for a graduate of an accredited registered nursing program or licensed social work program (BSN or LSW degree), but a Bachelor of Science or Bachelor of Arts in a related field may be considered.
- Possession of Licensure in State of Illinois if applicable.
- Experience in management within a healthcare setting preferred.
- Knowledge of Joint Commission standards and healthcare compliance requirements preferred.
- Proficiency in database management and reporting.
- Bilingual in Spanish preferred, but not required.

Pay:
$58, $67,304.00 per year
