Job Summary:
Responsibilities:
- Conduct comprehensive patient assessments
- Develop and implement patient care plans
- Perform procedures such as suturing, catheter care, and dialysis
- Manage pain and provide palliative care
- Engage in discharge planning and coordination of care
- Specialize in memory care for patients with cognitive impairments
- Utilize telehealth technologies for remote patient monitoring
- Participate in clinical research studies
- Utilize Epic electronic health records system
Qualifications:
- Master's or Doctorate degree in Nursing
- Current Nurse Practitioner license in the state of practice
- Certification as a Nurse Practitioner in the respective specialty area
- Proficiency in patient assessment and diagnosis
- Strong clinical skills including suturing and catheter care
- Experience in pain management and palliative care
- Knowledge of memory care practices for patients with cognitive issues
- Ability to conduct telehealth consultations effectively
- Familiarity with Epic EHR system is a plus
- Interest or experience in clinical research is desirable
