- Impact lives by conducting Adult/Geriatric Assessments, medical history reviews, diagnosis and treatment, health education, physician referrals, case management referrals, and clear documentation following Matrix guidelines and protocols
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and other medical personnel as needed
- Work with Primary Care Physicians (PCPs) on patient education and follow-up care
- Network with advanced practice providers nationwide to establish and nurture relationships
- Services may be offered in various settings including Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree required OR relevant experience and NP licensure completion
- Current RN and NP licensure in the state of practice, including prescription authority or eligibility for prescriptive authority
- Board certified by the AANP, ANCC, or the AACCN in a Matrix-approved specialty
- Current BLS, ACLS, or CPR certification
- Minimum of 1 year of experience as a Nurse Practitioner
- Strong computer skills and familiarity with health/medical record software
- Excellent verbal and written communication skills with patients, clients, and colleagues
- Adaptability and comfort with frequent changes
- Ability to travel, possess a valid state driver's license, and capability to drive with appropriate automobile insurance coverage
- Travel may be necessary

About the Position:
Matrix providers conduct in-depth assessments during visits that can last up to one hour, reviewing a member's current health, medical history, medication adherence, social environment, and other risks. This offers valuable insight into overall health and well-being that is often challenging to capture during routine office visits. The Matrix Comprehensive Health Assessment enhances care quality and allows us to potentially address multiple care gaps in a single visit.

Matrix providers review a member's health, medical history, medication adherence, social environment, and other risks during visits that can last up to one hour. This insights into the member's overall health and well-being can be challenging to obtain during routine office visits. The Matrix Comprehensive Health Assessment assists in enhancing care quality and potentially addressing multiple care gaps in a single visit.
Nurse Practitioner - PRN - Home Risk Assessments
Type: PRN-Flexible-Nurse Practitioner
Location: Home, SNF, Telehealth, and other community settings in Charleston, Mississippi
Hours: Days, Evenings, Nights, Weekends, Flexible Scheduling
PAY: $90.00 per completed Assessment, 25 hours or more/month - Flexible Scheduling
Benefits: PRN providers receive limited Voluntary Benefits
Mileage Reimbursement provided for all miles when 20 or more assessments are completed per month, No-show payment for 10 or more booked assessments per month
Matrix Provider - What to Expect:
