    Job DescriptionMinimum 1+ years nursing experience required.

    We are currently seeking a dedicated and compassionate Contract Home Health Registered Nurse (RN) to join our team in Seattle. As a vital member of our healthcare organization, you will play a crucial role in providing high-quality, patient-centered care to individuals in the comfort of their homes.

    Responsibilities:

    1. Conduct comprehensive assessments of patients' health status.
    2. Develop and implement individualized care plans in collaboration with the patient and their healthcare team.
    3. Administer medications, perform wound care, and provide skilled nursing care as needed.
    4. Monitor patients' progress and adjust care plans accordingly.
    5. Educate patients and their families on proper healthcare techniques and disease management.
    6. Collaborate with other healthcare professionals to ensure coordinated and holistic patient care.
    7. Maintain accurate and up-to-date patient records.

    Qualifications:

    1. Current and unrestricted Registered Nurse (RN) license.
    2. Minimum of 1yearof experience in home health.
    3. OASIS experience is required.
    4. Excellent clinical assessment and critical thinking skills.
    5. Strong communication and interpersonal skills.
    6. Ability to work independently and as part of a collaborative team.
    7. Reliable transportation and a valid driver's license.
