Homecare is seeking a creole intake specialist to join their team


The Intake Specialist will be responsible for conducting client intake assessments, providing support services, and connecting clients with appropriate resources.


Responsibilities for Creole intake specialist:

  • Conduct intake interviews with clients to assess needs and determine eligibility for services
  • Provide information and referrals to clients based on their individual needs
  • Assist clients in completing necessary paperwork and documentation
  • Maintain accurate and uptodate client records in compliance with regulations
  • Collaborate with internal teams to ensure seamless service delivery
  • Follow up with clients to monitor progress and address any concerns

Qualifications for Creole intake specialist:

  • Intake experience
  • MUST be fluent in Creole

Pay:
$ $25.00 per hour


Schedule:

  • Monday to Friday

  • Intake: 5 years (preferred)

Ability to Relocate:

  • Brooklyn, NY: Relocate before starting work (required)
