    Job Description

    The Transplant Coordinator is a Registered Professional Nurse with specific knowledge and skills who participates in the coordination of clinical programs and services for designated patient populations within the Transplantation Institute.

    Responsibilities

    1. Coordinates the multidisciplinary screening of patients being followed in the transplant clinic. This includes the transplant director and surgeons, nephrologists, tissue typing laboratories, urology services and ancillary services.
    2. Ensures all necessary lab tests are performed on potential transplant candidates and that results are updated as required.
    3. Participates in making recommendations regarding placing as patient on the transplant waiting list.
    4. Communicates with all active transplant candidates regarding waiting list status on an ongoing basis.
    5. Assists in organ procurement team with donor screening and procurement coordination, especially at MSMC and satellite.
    6. Follows patients during their hospitalization and coordinates works with transplant team to coordinate consulting services as needed
    7. Prepares patient and family for transition to home. Supervises instructions regarding medication, diet, exercise, contraindications and outpatient follow-up protocols.
    8. Assesses continued recover of patient through visits, scheduling of blood tests, E K Gs, comprehensive evaluations, etc.
    9. Supports the mission, philosophy, goals and objectives of the Department of Nursing and The Mount Sinai Medical Center.
    10. Maintains clinical competence in this advanced specialization of nursing practice.
    11. Is responsible and accountable for his/her own nursing practice.
    12. Advocates the highest standards of nursing practice.
    13. Approaches patients, families and other members of the health care team in a professional, respectful manner.
    14. Serves as a clinical role model and resource to the nursing staff
    15. Serves as a professional resource to other members of the health care team.
    16. Demonstrates professional involvement by:
    Attending and participating in workshops, seminars, and courses as presenter or author;
    Keeping abreast of current literature and trends in practice; Participating in related professional organizations
    17. Considers ethical issues in professional nursing practice.
    18. Treats co-workers, patients and families with dignity and respect.
    19. Is open and responsive to the diverse backgrounds and experience of other people, and promotes and environment that is sensitive to cultural diversity.
    20. Maintains attendance and punctuality record in accordance with departmental standards.
    21. Adheres to dress code in accordance with departmental policy.
    22. Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations.
    23. Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards.
    24. Participates in the development and implementation of continuous quality improvement programs.
    25. Participates in risk management activities including identifying and communicating issues of risk in a timely manner.
    26. Participates in committee activities.
    27. Is aware of and responsive to budgetary constraints and incorporates financial principles into program development and allocation of resources.
    28. Recognizes each individual's right to health care unrestricted by age, race, color, sex, age, national origin, disability, sexual orientation or source of payment.
    29. Assists in the performance appraisal process of designated Nursing

    Qualifications

    NYS RN with BSN and BCLS, and at least 2 years of acute care nursing experience, critical care experience and/or specific experience with patient population preferred. Proven progressive responsibilities is essential.
