Description

In this role, you will be responsible for providing comprehensive nursing assessments, developing personalized care plans, and coordinating with multidisciplinary healthcare teams to ensure high-quality home health services for our clients.

• Conduct thorough in-home assessments of clients' physical, psychological, and social needs.
• Develop, implement, and evaluate individualized care plans in collaboration with clients, families, and healthcare team members.
• Monitor clients' progress, identify potential health issues, and adjust care plans accordingly.
• Educate clients and their families on disease prevention, health promotion, and self-care strategies.
• Coordinate care services with physicians, therapists, and other healthcare professionals to ensure optimal client outcomes.
• Maintain accurate documentation of assessments, interventions, and client responses in accordance with regulatory guidelines.
• Stay current with the latest evidence-based practices, research, and technological advancements in home health care.
• Participate in quality improvement initiatives and contribute to the overall success of the home health agency.

• Registered Nurse (RN) with an active, unencumbered nursing license in the state of Illinois.
• A minimum of 2 years of experience in a home health or community health setting.
• Strong knowledge of nursing principles, practices, and techniques.
• Proficient in conducting comprehensive health assessments and developing effective care plans.
• Excellent interpersonal, communication, and organizational skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Familiarity with electronic health records and documentation systems (Axxess).
• Current Basic Life Support (BLS) and CPR certification.
• Valid driver's license and reliable transportation for home visits.

• Certification in Home Health Nursing (e.g., RN-BC, HHCN)
• Bachelor's degree in Nursing (BSN)

• Proven experience as a registered nurse
• Excellent knowledge of nursing care methods and procedures
• Excellent knowledge of emergency care
• Indepth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
