    Exciting tasks you'll undertake:
    This role involves planning, coordinating, and delivering quality skilled services. The Care Team Manager is responsible for orienting and supervising direct care staff, as well as supporting and coordinating all services. Additionally, the Care Team Manager may step in for the Administrator as needed.
    • Manage the patient care team to ensure compliance with state and federal requirements.
    • Facilitate communication and coordination of care between various parties involved.
    • Coordinate care efficiently through case conferences and scheduling processes.
    • Participate in on-call responsibilities as required.
    • Maintain accurate clinical records in compliance with regulations.
    • Perform OASIS Assessments and skilled visits when necessary.
    • Review patient intake information to ensure coverage criteria are met.
    • Complete other duties as assigned.
