- Provides quick and accurate clinical assessments of mental health and substance abuse conditions.
- Provides brief, solution focused therapeutic engagement with patients as needed.
- Provides assistance in detecting and addressing behavioral health needs within the primary care setting.
- Coordinates and facilitates communication between primary care physician and psychiatrist when needed.
- Utilizes behavioral health expertise to promote change to improve behavioral and physical health conditions.
- Provides referral and community resource information.
- Assists with linkage to more intense psychiatric care as appropriate.
- Performs other related job duties as assigned.
- Masters Degree in Social Work or Counseling.
- 2 plus year of clinical experience.
- 1 plus year of experience using Microsoft Professional Office Suite, to include Word, Excel, PowerPoint and Outlook. Required Licensure/Certification/Specialized Training:
- Licensure as an LMSW, LCSW, or LPC.
- Certification in Non-Aggressive Crisis Management; must be obtained while in orientation and maintained throughout employment.
- BCLS certification required but may be obtained subsequent to hire.
