Position Summary


Under the supervision of the QIDP of the home and the ARTD, the Lead House Services Coordinator is responsible for the coordination and implementation of services for group homes where adults with developmental disabilities reside.

This position is responsible for ensuring the needs of the individuals living in the assigned group homes are met.

The Lead House Services Coordinator is also responsible for ensuring basic systems are in place and operating successfully within the assigned residential site.

This includes, but is not limited to, medication administration, laundry, shopping, menu/meal planning and maintenance and safety of the group home sites and assists with maintaining regulatory compliance.

The position interfaces with staff, peers, management, families, neighbors, medical practitioners, Regional Center Service Coordinators and Regulatory agencies.


Responsibilities:


  • Manage staff's payroll
  • Process biweekly payroll, track staff attendance and review their time off request.
  • Ensure payroll is processed timely and accurately.
  • In partnership with QIDP, manage and supervise HSC's and Residential aides.
  • Responsible for conducting interviews and notifying the QIDP of any staffing need.
  • Responsible for the group home's staff schedules ensuring the home is operating per required ratios. Manages scheduling for all staffing needs, scheduling changes including call outs, and other scheduling changes, while avoiding unneeded overtime.
  • Directly responsible for filling call outs, up to and including working the shift.
  • Ensure required direct care staff training is provided on an ongoing basis.
  • Train staff on implementing goals and intervention techniques, to ensure that the procedure is consistently and systematically implemented across staff.
  • Responsible for ensuring all staff obtain and maintain valid certifications, i.e. CPR/First Aid, Medication Administration, Driver Safety Training, Water Safety, etc.
  • Responsible for assigning, scheduling and implementing monthly house meetings and ensuring full attendance.
  • Responsible for the development and implementation of household schedules, in partnership with the QIDP.
  • Facilitate the implementation of the daily household schedule, laundry schedule, and activity schedule, that ensures maximum involvement of residents and that individual needs are met through active treatment.
  • Assist with client selfcare and provide specialized, intensive independent living skills training in the area of selfcare, personal hygiene, medical and health needs, nutrition, care of clothing, money management, household maintenance, etc.
  • Works with staff to ensure residents' goals are implemented properly and client data is input timely into client medical record or other documentation process as outlined in the client person center plan/IPP. Makes goal recommendations to the QIDP and consultants.
  • Responsible for ensuring all client data sheets are printed and put in the binder by the first of every month.
  • Ensure the client behavior and IPP goals data is tallied and submitted to the QIDP by the third of each month.
  • Meets with direct support staff team monthly; documents monthly meeting minutes and provides the minutes to the QIDP.
  • Observes direct support staff individually each month via spot checks, to support and train them on essential job duties, agency policies and procedures, and regulatory requirements.
  • Provides documentation of observation and any inservices completed during spot checks to the QIDP within 3 working days.
  • Provide new hire orientation to new staff working in the home.
  • Enforces agency training mandates.
  • Works in cooperation with staff assigned to individuals to ensure resident's successful participation in appropriate recreational and community activities.
  • Facilitates communication and relationship building for residents.
  • Processes all incident, accident reports and human rights issues in a timely manner according to agency policy/procedures and licensing regulations.
  • Ensures human rights of individuals are safeguarded in all interventions. Along with QIDP, responsible for ensuring all staff are following behavioral intervention plans.
  • Ensures the medical services and records meet the needs of the individuals and conform to regulations.
  • Provides crisis intervention and emergency coverage for assigned site. After consulting with QIDP, secures additional resources as needed, identifies and resolves programs that resulted in an emergency situation. Performs direct support related tasks in emergencies or as requested by QIDP.
  • Provides transportation for residents as needed.
  • Under the direction of QIDP, assists in the preparation of site and individual records for DDS licensing and to meet the standards of quality established by the agency and funding sources.
  • Properly oversees medication administration
