About the Community Services Coordinator Position


The Community Services Coordinator is responsible for assigning volunteers for the agency, as well as coordinating the program functions and customer-focused services of the Community Services Department.

This position works in collaboration with the respective department managers.

The Community Services Coordinator provides program coordination, support and planning for community services including Grief Support, Pedi Pal, and Caregiver programs.

This is a full-time, 40-hours per week position that is eligible for full benefits.

This is a full time position with the hours of 8:00am-4:30pm and it is eligible for benefits


What will you do as a Community Services Coordinator?

  • Coordinates and assigns volunteer services to patients, family members, and HopeHealth staff that are consistent with the goals, definition, philosophy, and purpose of HopeHealth
  • Provides first line customer service to programs within Community Services department by responding to customer calls. Provides information to callers or routes calls to appropriate personnel, takes messages, or relates information in a confidential, accurate, timely and professional manner.
  • Assists in the planning and implementation of yearround programming within the Community Services Department.
  • Acts as a liaison between the patient and/or caregiver, other internal departments such as finance, IT, and HIS, and external departments as relative to the programs within Community Services department, supporting all with correspondence and communications between parties to ensure seamless care.
  • Responsible for data management as it pertains to all community service program and prepares related internal and external reports and/or works with decision support to obtain reports ongoing and as needed. Performs data entry of demographics relating to community services department.
  • Collaborates with department managers, to support the creation and maintenance of all collateral and support materials internally and externally for Community Services Department. Submit new collateral to Community Resource Database for all community services departments.
  • Coordinate services with other program staff members and external vendors. Register callers for various community service programs as needed. Register attendees, create invoices, collect credit card, or check payments for community services programs. Submitting, tracking, and following up with submissions of invoices, and reimbursement receipts. Assist with set up and registration for community service events.
  • Monitors internal folders and documents for all community services departments, updating as needed and notifying staff of changes.
  • Coordinate grant disbursement for program specific recipients when needed.
  • Coordinate all tasks related to various ongoing mailings for community services department including bereavement monthly mailing and support group schedules. May coordinate work for office volunteers to assist with these tasks and others.
  • Special projects and other duties as requested by Senior Director of Clinical Outreach Services.

What are the qualifications for this job as a Community Services Program Coordinator?

  • HIgh school diploma required, Associates degree preferred but not required or equivalent experience in administrative support and coordination
  • Proficient skills using a computer are mandatory. Ability to be effectively trained to use various software for job related tasks.
  • Significant experience with data base programming at the input and report running end is required.
  • Knowledge of Excel (or similar spreadsheet) and Microsoft Word (or similar wordprocessing software) is required.
  • Knowledge and working ability with medical software is desirable.
  • Ability to evaluate situations according to established standards and policies. In addition, ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
