Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team.

As a Case Manager, you will be responsible for coordinating and providing comprehensive care to patients, ensuring their needs are met and that they receive the highest level of support.


Responsibilities:


  • Conduct thorough assessments of patients' needs, including physical, emotional, and social factors
  • Develop and implement individualized care plans based on assessment findings
  • Collaborate with healthcare professionals to coordinate and facilitate patient care
  • Provide patient education on disease management, treatment options, and available resources
  • Assist with discharge planning and ensure a smooth transition from hospital to home or other care facilities
  • Maintain accurate and uptodate medical records and documentation
  • Communicate with patients, families, and caregivers to address concerns and provide support
  • Advocate for patients' rights and ensure they receive appropriate services and resources
  • Collaborate with interdisciplinary teams to develop strategies for improving patient outcomes

Skills:


  • Strong knowledge of medical terminology and diagnostic evaluation processes
  • Experience in case management or related field preferred
  • Ability to effectively communicate with patients, families, and healthcare professionals
  • Proficient in using electronic medical record systems
  • Excellent organizational skills with the ability to prioritize tasks and manage time effectively
  • Knowledge of discharge planning processes and resources available for patients' posthospitalization needs
  • Familiarity with primary care principles and practices
  • Experience working with pediatric patients is a plus
