Position Title:
Health Services Coordinator

Scope:
The Health Services Coordinator reports directly to the Director of Residential Services.

The Health Services Coordinator is responsible for the overall medical management of individuals receiving services in UCP of NEPA's Community Residential Program.


Basic Function:

The Health Services Coordinator is primarily responsible for the overall functioning of the medical management of UCP of NEPA's residential community homes.

The Health Services Coordinator will be the official liaison between UCP of NEPA and medical professionals within the community.

The Health Services Coordinator will function as an active member in the interdisciplinary team (IDT), as well as serve as a role model for all staff and residents within the assigned community homes.


Position Schedule:
The Health Services Coordinator position is a full-time position that requires on-call duties for emergency medical situations. The base schedule for this position is Monday - Friday 8:30am - 4:30pm. _
Hours may vary based upon on-call responsibilities_


Position Qualifications:
Active Pennsylvania State Licensed Practical Nursing (LPN) in good standing.


Position Salary:
The Health Services Coordinator position is a full-time hourly position compensated at $31.00 per hour.


Essential Job Elements:

I. The Health Services Coordinator will plan, develop, implement, organize and document medical programs for their assigned residential caseload.

II.

The Health Services Coordinator will work collaboratively with direct care/supervisory staff to schedule and track annual appointments to include, but not limited to the following - physicals, eye exams, dental visits, psychological appointments, hearing evaluations etc.

III. The Health Services Coordinator will assist in routine health education programs with department staff.

IV. The Health Services Coordinator will assist in coordinating, tracking, as well as participating in residents' health appointments.

V.

The Health Services Coordinator will assume responsibility for hospital admission and discharges, ensuring appropriate documentation is completed and disseminated, and continuity of care protocol(s) are utilized (medication/equipment/etc.)



VI.

The Health Services Coordinator will be an active member of the Interdisciplinary Team (IDT), and will be present for resident annual and update meetings.

VII.

The Health Services Coordinator in conjunction with the Residential Coordinator will be responsible for implementing the residents' Individual Support Plan (ISP) as developed by the person-centered planning team.

VIII.

The Health Services Coordinator will ensure that daily and/or routine medical documentation is accurately completed by all staff within the residential community homes.

IX.

The Health Services Coordinator will ensure that all medical forms are filed within the residential community homes and at the Administrative Office.
