    Job DescriptionDescription:

    Southeast Primary Care Partners (SPCP) seeks a dedicated and dynamic value-based care leader to join our team. This pivotal role serves as the crucial link between the physicians and practice managers in the clinic and SEMG's population health team, with a focus on driving and improving quality outcomes for our patients, improving HEDIS/STARS, and improving clinical documentation. The ideal candidate will be instrumental in executing key initiatives aimed at enhancing performance on quality measures, actively leading monthly quality review meetings, coaching providers and managers on value-based care improvement opportunities, and identifying patients for enrollment in various clinical programs.

    Multiple positions are available in Alpharetta, Johns Creek, Cumming, Fayetteville, and Stockbridge, GA.

    Requirements:

    Requirements

    • Liaison Activities: Act as the primary liaison between the clinic and SEMG's population health team to facilitate improved quality outcomes.
    • Quality Initiatives Execution: Implement and monitor key initiatives within the clinic to enhance performance on quality measures. This includes outreach to patients overdue on care gaps, coordinating with other providers to gather medical records, and scheduling necessary appointments.
    • Monthly Quality Reviews: Engage actively in ongoing monthly quality review meetings with providers and the population health team, aimed at identifying and leveraging opportunities for improvement.
    • Patient Enrollment: Identify patients eligible for and assist in enrolling them into SEMG's clinical programs, such as care management and remote monitoring services.
    • Other duties as assigned.

    Minimum Qualifications:

    Greater than 3yrs experience with:

    • Value-Based Payor Programs (incl. understanding of HEDIS Coding)
    • HEDIS, STARS ratings and other population health initiatives.
    • Electronic Health Record (eHR) use and documentation. Allscripts/Veradigm experience preferred.
    • Demonstrated experience training clinical staff on value-based programs and requirements
    • Demonstrated experience with auditing documents for gap-closure requirements
    • Associates Degree preferred (healthcare or business related)

    Physical & Mental Requirements:

    • Ability to sit for extended periods of time
    • Ability to stand for extended periods of time
    • Ability to use fine motor skills to operate office equipment and/or machinery
    • Ability to receive and comprehend instructions verbally and/or in writing
    • Ability to use logical reasoning for simple and complex problem solving
    • Ability to travel to multiple locations to support business needs as required
