- Act as the central point of contact between caregivers and patients to ensure continuous, high-quality care.
- Build and nurture relationships with vendors to support patient census growth.
- Ensure compliance with VBP Measures and quality standards.
- Strategically schedule services for clients and caregivers.
- Facilitate transparent communication within the department regarding scheduling, attendance, and other significant issues.
- Verify daily caregiver attendance and prioritize staffing cases based on patient needs.
- Keep stakeholders informed of any patient care issues and maintain updated documentation.
- Handle inquiries from physicians, contracts, and clients promptly and professionally.
- Maintain patient confidentiality and dignity throughout all interactions.
- Handle non-clinical patient and caregiver queries efficiently.
- Document patient care activities accurately and per department procedures.
- Fluency in English, Korean, and either Cantonese or Mandarin.
- Strong verbal and written communication skills.
- Exceptional interpersonal and customer service abilities, especially with elderly individuals.
- Capability to manage challenging situations with empathy and professionalism.
- Ability to perform effectively in a fast-paced and occasionally stressful environment.
- Experience in utilizing HHA Exchange platform is advantageous.
