About the Job:

We are looking for someone to join our team as Client Services Coordinator. As the Client Services Coordinator, you will work closely with the Director of Outpatient Services, acting as a key source of information and support for program participants seeking social services and care from community providers.

Responsibilities:

Provide case management services to clients, assisting with entitlements, housing, and medical services.
Build and maintain strong relationships with clients, supervisors, and other stakeholders.
Ensure compliance with all relevant rules, regulations, and policies.
Maintain accurate records and documentation.
Collaborate with other programs within the organization.
Act as a liaison between social service agencies and our program.
Attend meetings and training sessions to stay updated on best practices.
Monitor patient compliance with treatment plans.
Drive as needed for business purposes.
Participate in quality assurance activities.
Stay informed about industry trends and developments.

Requirements:

Bachelor's degree in Social Work, Psychology, Sociology, or related field.
Minimum of 1 year of experience in human or social services.
Valid driver's license and clean driving record.
Pass required background checks and drug screening.
*Equivalent combination of experience and education considered.

Physical Demands & Environment:

Work takes place in psychiatric and correctional facilities.
May involve exposure to biohazards and potential risks.
Physical activities include standing, walking, bending, lifting, and more.
Must have normal sensory abilities for the role.
