Summary Description of Work:

The Heartland Housing Solutions Program offers scattered site Permanent Supportive Housing through long-term financial aid and supportive services, using a Housing First model, for individuals facing chronic homelessness in Douglas, Sarpy, and Pottawattamie Counties.
This program focuses on individuals and families with prolonged episodes of homelessness and high service needs.
The Case Manager will assess eligibility, help clients find and secure housing, and provide long-term case management/supportive services.

Compensation:
between $19.74 and $23.54 per hour (rate determined by total relevant experience)

Work Schedule:
Monday to Thursday, 8:30 a.m. to 5:00 p.m. and Friday 8:30 a.m. to 2:30 p.m.

Minimum Qualifications:

Bachelor's Degree in Social Work or a Human Service field, plus two years of direct care experience in Human Services preferred; or equivalent
Valid Driver's License and acceptable driving record
Key Responsibilities:
- Conduct assessments to accept, deny, or refer applicants
- Verify and document applicant eligibility and maintain comprehensive service records
- Assist individuals in locating, securing, and maintaining suitable housing
- Develop housing stability plans with clients
- Help clients improve financial security through employment or benefits
- Provide planned and crisis response services
- Teach clients communication and self-advocacy skills
- Ensure safety and security measures are followed during in-home services
- Maintain positive relationships with landlords, clients, and referral sources
- Offer problem solving and crisis intervention services
- Evaluate community service providers' efficiency and effectiveness
- Participate in internal and external training opportunities for skill development
- Contribute to fundraising and friend-raising efforts of the agency
- Travel may be required, including the use of a personal vehicle

This job involves a high level of social work skills and knowledge, decision-making, problem-solving, and interpersonal skills. It necessitates the use of trauma-informed and culturally competent practices in client service delivery.
