_ Full-time role - Monday-Friday_
***: The Hospice Nurse Practitioner in collaboration with the Hospice Medical Director and the Director of Patient Care Services provides advanced hospice care services to patients and their families. The Nurse Practitioner will support face-to-face encounters for hospice patient re-certifications, obtain medical histories, conduct physical exams, and educate patients and families in the disease process. This position requires the ability to manage administrative performance; expertise in oral and written communication, staff supervision, managing documentation and service coordination.


QUALIFICATIONS:


Education:
Master's degree in Nursing from an accredited school of nursing.


Credentials:
Current CA Nurse Practitioner License


Experience:
Minimum of 3 years of hospice and/or palliative care setting experience at an advance level preferred. Must possess sound judgment, organizational skills and the ability to prioritize and follow-through accurately and efficiently. Computer skills are required.
