- Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function
- Administer tests and measurements to determine functional goals and outcomes
- Create a comprehensive evaluation and care plan outlining physical therapy needs, goals, progress, and expected results
- Deliver physical therapy services in accordance with prescribed physician orders
- Maintain care plans through timely documentation, adjustment of goals, and appropriate discharge planning
- Collaborate with the healthcare team, physicians, and caregivers to ensure coordinated services
- Ensure thorough documentation to meet regulatory standards and facilitate effective communication among team members
- Supervise other personnel as required and complete OASIS assessments where permitted
- Adhere to policies, procedures, and regulatory requirements to uphold quality patient care standards
- Graduate of an accredited Physical Therapy program
- Current, unrestricted state licensure as a Physical Therapist
- Valid CPR certification and Driver's License
- Strong organizational and communication abilities
- Attention to detail and effective time management skills
- Proven track record in building relationships
- Experience in Home Health Therapy and proficiency in OASIS documentation
