Who We Need


The Health and Wellness Coach provides a range of evidence-based and/or certified programs, services and interventions designed to assist service participants in developing and fulfilling their personal health and wellness goals.

Assists the primary care provider in linking participants with health risk factors to direct care and community services including fitness, nutrition, smoking cessation, stress management and personal recovery resources.

Serves as a health advocate and navigator to assist participants and their families with their self-management goals.


JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO)

  • The Health and Wellness Coach is responsible for conducting outreach and contacting participants to engage them in health and wellness services, including managing direct referrals from the primary care provider and BHMPs. Manages referral lists and participant signin logs.
  • The Health and Wellness Coach will focus on areas of wellness success such as: Chronic Disease Management, Physical Activity, General Wellness Nutrition, Stress Management, Tobacco Recovery, and Weight Management.
  • Supports the clinical care team and primary care provider in maintaining a focus on whole person health and wellness through, motivation, information/education, and celebrating participant successes.
  • Obtain necessary fitness/diet clearances and informed consents for participation in health and wellness programs.
  • As applicable, maintains receipts and invoices for program purchasing and supplies.
  • Performs other comparable duties as assigned.
  • Maintains the fitness center at the clinic (if applicable), facilitating fitness related groups and one on one opportunities for Members.
  • Ability to meet specific productivity goals monthly based on billing standards.
  • To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department.

MINIMUM QUALIFICATIONS:


  • Bachelor's degree in the fields of wellness, fitness, exercise, health or nutrition, including but not limited to a bachelor's in Health Sciences, Nutritional Sciences, Health Promotion and Exercise Sciences, with at least one (1) year of work experience in the Health Promotion field
  • Behavioral Health Technician with two (2) years' work experience in health and wellness, fitness, nutrition, physical activity and/or personal training.
A qualified Behavioral Health Technician has one of the following:

  • At least a bachelor's degree in a field related to behavioral health;
  • Registered nurse
  • Physician assistant who is not working as a medical practitioner;
  • A bachelor's degree in nonbehavioral health related field and at least one year of fulltime behavioral health work experience;
  • A high school diploma or high school equivalency diploma and four (4) years of fulltime behavioral health work experience; or
  • Is licensed as a practical nurse, according to A.R.S. Title 32, Chapter 15, with at least two (2) years of fulltime behavioral health work experience
  • Satisfactory work record as determined by the People First/Human Resources Department.
  • Successful completion of Copa's training program.
  • Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
  • Successfully complete a preemployment drug screening.
  • TB Test clearance required at the time of hire and annually.
  • Must have reliable transportation, valid AZ Driver's License with state minimum insurance coverage and ability to meet and maintain Copa Health's acceptable driving requirements.
