Medical Assistant

Dr. Prather & Lifestyle Medicine (LSM) Program

Full-Time

Summary:

The Medical Assistant will support Dr.

Heidi Prather's practice and the LSM program, primarily located at HSS Manhattan locations, and will meet with the interprofessional team members regularly.


Key Responsibilities

  • Patient Management_
  • Serve as the consistent clinical resource for patient questions and concerns for both patients and staff.
  • Check patients in for in person and virtual visits to ensure consistency in recording intake measures needed for the LSM program including Blood pressure, weight, labs, patientreported outcome measures (PROMs).
  • Place patient calls to answer patient questions and serve as a liaison to LSM primary providers
  • Manage patient and collaborating provider requests in the EPIC portal.
  • Educate patients and encourage adherence to their plans of care.
  • Collaborate and consult with the care team to facilitate the patient journey and achieve desired outcomes and patient goals.
  • Clinical Team Coordination_
  • Develop relationships with various clinical teams within HSS including Physical Therapy, Registered Dietitians, Exercise Physiology, Social Work, and Endocrinology departments and serve as the clinical liaison to the medical director and nurse practitioner.
  • Work closely with the LSM Care Navigator to facilitate patient needs
  • Work with the interprofessional team members to answer patient questions and facilitate care needs.
  • Participate and contribute to the monthly interprofessional team meetings.
  • Administrative Management_
  • Manage clinical information from patients and providers including imaging, laboratory, and medical records to insure inclusion in the patients HSS medical record.
  • Answer medical questions as needed for the preaccess scheduling team.
  • Provide insight into the care management process and work with administration to ensure patients can receive the care they need in a timely manner.
  • Maintaining clear and effective communication with all applicable program stakeholders.

Qualifications:

Requirements - Required

  • Medical assistant certification
  • Previous healthcare work experience
  • Ability to prioritize issues, resolve conflicts and communicate with all levels of the organization.
  • Team oriented and energized by serving as a major contributor to scaling care management offerings for ambulatory patients at HSS.
  • Must possess ability to learn new software tools & systems as required on the job.
  • Initiativetaking, detailoriented, and pragmatic; energized by working in a fastpaced, evolving environment.

Requirements - Preferred

  • Previous work in EPIC

Other Requirements:
#LI-Onsite Pay Range - Minimum: USD $30.77/Hr


Pay Range - Maximum:
USD $31.72/Hr.
