Position:
Social Services Coordinator - Council on Aging

Category of Employment:
Full time 35 hrs/week

Pay Grade and Range: 8 ($ /hr)


Summary
This position is to provide professional and direct social services to the community's Senior Citizens.


Essential Function

  • Assess and facilitate services and programs for Elders in Harwich who may be unserved or underserved and monitor any changes in the individual's situation.
  • Coordinate with the Town of Harwich Public Safety Agencies, R.E.A.C.H Coordinator and program, other community agencies such as Pine Oaks Villages and other senior housing as well as with Elder Services of Cape Cod and the Islands.
  • Assist with Fuel Assistance & Food Stamp Applications & renewals and make referrals as necessary. Will be responsible for handling crisis intervention and will coordinate with the COA Director and the Town Nurse as needed.
  • Must be organized, detailoriented and flexible with the ability to prioritize and multitask.
  • Identify the need and develop new programs for community outreach services.
  • Provides outreach social services to the Senior Citizens of Harwich.
  • Works independently and in conjunction with other human service provider agencies.
  • Provides advocacy and works in conjunction with human service agencies and specialists.
  • Maintains confidential client records.
  • Prepares monthly reports to the Council on Aging detailing outreach activities and services. Prepares annual report for Executive Office of Elder Affairs.
  • Meets monthly with various agencies to review caseloads, and makes recommendations to improve the quality of life of the Seniors.
  • Coordinate with COA staff in grant request.
  • Participate in Bi-Monthly COA Staff Meetings.

Certification and Licenses
Must possess or be able to obtain and maintain a valid Massachusetts driver's license.


Education and Experience


Bachelor's Degree in social work, Gerontology or human services and three-five years' experience working with the elderly in a social service environment or related field.

Proficient in Microsoft Word and Excel. Database experience a plus. CPR/AED required.


Knowledge, Skills and Abilities

  • Thorough knowledge of the resources available to assist the elderly, and the procedures necessary to provide such assistance.
  • Thorough knowledge of State Statutes and eligibility requirements for public assistance as well as legislation relating to the aging.
  • Thorough knowledge of the philosophy, principles and practices of public senior services administration.
  • Thorough knowledge of the principles and practices of modern professional and social work methods.
  • Thorough knowledge of State Statutes and eligibility requirements for public assistance as well as legislation relating to the aging.

Pay:
$ $35.88 per hour
