Responsibilities:
- Supervise 4+ Senior Case Mangers
- Provide training to new staff members
- Directs case managers in developing corrective action plans in ongoing case management files, incident reports and inhome visit reports
- Conduct new case assignments for social services staff caseload, including reassignments of terminated/resigned employees
- Lead monthly group supervision and case conferences on weekly basis on key client cases, maintain documentation of provided supervision sessions
- Supervise staff and volunteers on telephone reassurance, escort, friendly visiting and coordinate recordkeeping of services and document their activity if necessary
- Assist Social Work staff with referral processes, crisis intervention, coordination of care, and connections to community providers
- Review case records to ensure that services are provided according to the assessed care plan and DFTA standards
- Participate in DFTA program assessments (audits), improvement plans, and corrective action plans
- Assist in development and revision of COPO's policies and procedures
- Analyze program data to identify trends, areas for improvement, and report these findings to senior management.
- Provide clinical support and guidance to Aging Services social work staff, and interns
- Supervise and train MSW interns for the agency/program/clients
- Lead and participate in professional development workshops and seminars to stay current with industry standards and advancements.
- Develop and maintain a comprehensive resource library for staff and clients, including relevant literature, tools, and community resources.
- Collaborate with Older Adults Program Director staff for efficient and effective site operation and coordination of services.
- Manage Adult Protective Services clients as needed
- Conduct HV as needed
Qualifications:
- Master's degree in Social Work or a related field preferred.
- Proven experience working with older adults.
- Supervisory experience or at least 5 years of field experience.
- Familiarity with DFTA and DFTA Senior Centers strongly preferred.
- Demonstrated experience in program development.
- Bilingual abilities preferred.
- Excellent oral and written communication skills.
- Ability to work both independently and as part of a team.
- Strong organizational skills with the capacity to take initiative, problem solve, prioritize duties, and adapt in a fastpaced environment.
- Attention to detail and ability to meet deadlines.
- Knowledge of Excel, Word, and STARS/Peer Place Program preferred
- Ability to critically analyze data and prepare comprehensive reports.
- Commitment to ongoing professional development and learning.
- Cultural competency and sensitivity to the needs of a diverse client population.
Pay:
$65, $75,000.00 per year
Schedule:
- 8 hour shift
- Monday to Friday
Ability to Relocate:
- Brooklyn, NY 11230: Relocate before starting work (required)
