They will identify obstacles to successful treatment and assist family to reduce these barriers via referrals, advocacy, linkages to community supports, and creative problem solving.
The position may also include general community education and support services. Prepares requisitions ordering laboratory tests, x-ray exams, blood tests, etc. and scheduling appointments. Performs case data entry in the EMR system. Schedules and coordinates dates between patients, physicians, and hospital, if applicable. Schedules other diagnostic testing when necessary.Education:
High School graduate or GED required. Bachelor#s degree social services / human services or case management certification preferred.
Licensure:
None Experience:
Community care work experience. Experience in Human Services.
Skills and Abilities:
Bilingual a plus. Ability to communicate with a variety of people.
Ability to utilize sound interpersonal skills when interacting and taking into consideration cultural background, linguistic skills and age of customers.
