- Accompanies individuals to medical appointments and activities
- Assists with daily living activities as required
- Enters data using Therap (EHR)
- Follows medically prescribed diets and exercise plans
- Completes essential documentation for program operations promptly
- Valid NJ State Practical Nurse License
- High School diploma or equivalent
- Valid and insurable driver's license in good standing
- Experience in g-tube feedings
- Preferred experience in working with intellectual, developmental, and related disabilities
