Ensuring quality of life through empowerment, safety awareness and respect and dignity. Monitoring their overall health and well-being in their daily living activities. Providing comprehensive intake evaluations, care coordination, assistance with ADLs, and individual, family, couples, and/or group treatment.Providing individuals and their families with clinical support regarding behavioral health and substance use issues to implement treatment plan goals.
Helping individuals and their families maintain and enhance their social support network with family through outreach in client homes, hospital, and other community settings.
The Program for Assertive Community Treatment (PACT) program is a highly integrated outreach and 24/7 residential program that helps five individuals meet their goals and reduce hospitalizations.
PACT provides clinical wraparound support in the community and in our group residential program to perpetuate recovery and wellness for the individuals served.
The PACT team is forensic-focused and designed to reduce arrests and incarcerations.Master's Degree in Social Work, Psychology, or Mental Health Counseling (required). Commitment to empowerment and growth, human rights, and community integration of individuals served (required). Working knowledge of community resources and how to help clients access them.
