Job OVERVIEW:


To work as a proactive member of APRI Team, providing meaningful individualized treatment services and support that encourage children with special health care needs and their families to experience personal growth and reach their fullest potential.


Essential Responsibilities:

To provide supports and services consistent with those outlines in the HBTS Plan and Individual profile. Services and supports include but are not limited to areas outlined below.

  • To ensure that all children with special health care needs and families supported are treated with Dignity and Respect.
  • To support children with special health care needs and their families in developing a positive self image.
  • To treat each individual within the family as person who have unique needs and wants.
  • To support children with special health care needs and their families to function to there fullest potential.
  • To support children with special health car needs and their families to participate in their home and community in ways that are meaningful to them.
  • To encourage children with special health care needs and their families to maintain positive relationships.
  • To work with HBTS support team in developing consistent and integrated support techniques, methods and programs.
  • To attend and actively participate in team meetings.
  • To support and care out all team decisions consistently and appropriately.
  • To attend inservice trainings as determined by the Program Director
  • To gather and submit data related to achieving HBTS goals in a timely manner.

  • Based Workers are responsible to maintain the following complete and legible contemporaneous records.
  • Bi weekly time sheets which are signed daily by the parent and staff
  • Daily documentation sheets which specifies the date, time location, duration and levels of participation of parent and child for each therapeutic objective
  • Individual scoring (data) sheets for each objective listed in the child's treatment plan. Data is compiled daily and documents the child's response to treatment, the level of assistance provided by staff or parents, and all other objective information identified by the therapeutic consultant as relevant to the evaluation of care for each objective.
  • Bi weekly case presentation sheets that prepare the staff for a supervisory meeting with clinical staff. Case presentations provide staff an opportunity to summarize data and other relevant information for each goal. These documents are also used to record feedback and recommendations provided by clinical staff.
  • Staff meeting once a month mandatory.
  • Meet with your clinical supervisor once a month.
  • To complete preparation materials and participate in the semiannual planning meetings, if requested (HBTS status update and renewal, IEP).
  • To ensure the optimal health and safety of children with special health care needs receiving supports.
  • To adhere to all safety policies, including safe driving and seatbelt policies.
  • All other duties as assigned and/or as identified in the HBTS treatment plan and Individual profile.

Qualifications:


  • 19 years of age
  • High School Diploma or GED required and 2 years experience working with children with special health care needs or a combination of experience, education, and training working with children with special health care needs and their families or an Associates Degree in a related field.
  • Proficiency using digital devices and operating systems
  • Willingness and ability to complete and perform CPR and Non-Violent Physical Crises Intervention (NCI).
  • Willingness and ability to complete HBTS Training Curriculum
  • Valid Drivers License and acceptable driving record required
  • Access to and provision of an insured automobile on an ongoing basis.

Essential Physical Requirements:


  • Lifting 50 pounds
  • Standing 70% of shift
  • Bending 50% of shift
  • Being in motion 75% of shift

Varies by needs of the individual child.
