This position is a part time, 25 hours per week, contract role (6 months).

Public Partnerships LLC supports individuals with disabilities or chronic illnesses and aging adults, to remain in their homes and communities and "self-direct" their own long-term home care.

Our role as the nation's largest and most experienced Financial Management Service provider is to assist those eligible Medicaid recipients to choose and pay for their own support workers and services within their state-approved personalized budget.

We are appointed by states and managed healthcare organizations to better serve more of their residents and members requiring long-term care and ensure the efficient use of taxpayer-funded services.

Duties & Responsibilities

  • Helps improve or strengthen the organization's image in the community by supporting our mission and philosophy.
  • Represents PPL and spreads education and awareness about the Self-Determination Program at community outreach events, including underserved communities.
  • Creates and finds opportunities to build awareness or support for the Self-Determination Program and PPL.
  • Connects and refers people to PPL to learn about FMS options that can best meet their needs.
  • Collaborates crossfunctionally with resources, community partners, case managers to facilitate even stronger service delivery and relationships in the community.
  • Ensures PPL is listed as a resource with community organizations, state organizations, or anywhere people are looking for FMS organizations.
  • Creates and delivers presentations and/or training to program participants, case managers, community organizations and other stakeholders.
  • Active participant in meetings and discussions about the Self-Determination Program, including ongoing advisory and stakeholders committee meetings.
  • Actively gathers information from stakeholders about the Self-Determination program to ensure PPL is delivering services in a way that meets the needs of consumers and participants.
  • Identifies gaps in services and brings solutions internally to streamline processes or make systems better for stakeholders.
  • Assesses and assists in utilizing available data and how it may be used in the context of our community presence.
  • Provides linkages and referrals from other communityfocused programs and organizations to PPL.
  • Collaborates internally to enhance programming for consumers and participants.
Attends regional events as necessary to build the organization's brand across the state.

  • Ability to attend events around Southern California on weekends and evenings with the ability to have a flexible schedule.
  • Knowledge of developmental disability services in California.
  • Strong written and verbal communication skills.
  • Proficient in MS Office Suite.

Qualifications:


Education:

A Bachelor's degree in Social Work, Human Services, or Sociology strongly preferred


Experience:

4+ years of relevant experience in community engagement efforts related to delivering self-determination or resources to diverse and underserved communities; bilingual in Spanish and English required

The base pay $ $40.00/HR listed may vary depending on skills, experience, job-related knowledge, and location. Certain positions may also be eligible for a performance-based incentive as part of total compensation.
