To provide quality case management and qualified professional support and services to inpatient social workers as assigned.
Works with local agency and service providers to facilitate sending and receiving collateral information, treatment referrals, and ongoing communications to support patient's successful inpatient treatment and discharge.
QUALIFICATIONS:
Education/Experience:
A graduate of a college, or university with a Masters degree in a human services field and has one year of full-time, post graduate accumulated mental health experience with the population served and a substance abuse professional shall have one year of full-time post-graduate accumulated supervised experience in alcoholism and drug abuse counseling; OR A graduate of a college or university with a baccalaureate degree in a related human service field and has two years of full-time baccalaureate accumulated mental health experience with the population served and a substance abuse professional shall have two years of full-time post-graduate accumulated supervised experience in alcoholism and drug abuse counseling; OR A graduate of a college or university with a baccalaureate degree in a field not related to human services and four years of full-time, post-baccalaureate accumulated mental health experience with the population served and a substance abuse professional shall have four years of full-time post-graduate accumulated experience in alcoholism and drug abuse counseling
Licensure/Certification:
Current North Carolina Driver's LicenseCPI Certification (Can be obtained after hire)
Knowledge and Skills:
Knowledge of developmental psychology and needs of various populations.
Knowledge of short term/stabilization mental health treatment.
Knowledge of systems theory.
Professional verbal and written skills needed for communicating the patient status and treatment needs to payers, families, professionals, paraprofessionals employees of the hospital, and members of the patient's community.
Ability to maintain professional records.Ability to maintain patient confidentiality and high ethical standardsAbility to maintain professional relationships with patients, patient's family members, staff and community personnel.
Knowledge of appropriate assessments, diagnoses, and interventions with patients.Knowledge of milieu management.
Skills in preparing and maintaining appropriate medical record documentation.
