    Case Manager
    Part Time, Day Shift / Every other Saturday / Hybrid after Training

    Unit: Medical Surgical

    In 2004, in an ambitious push to bring hospitality back to the hospital, a small group of top Metro-Detroit surgeons decided to create their own surgical hospital; one that provided patients with the best possible care in a small, easy-to-manage environment that truly embraces the best patient experience. Our highly skilled surgeons and staff play a key role in our success rates and becoming the premier center of choice with 30 surgical beds, 26 rehabilitation beds, and 6 operating rooms.

    WHAT WE ARE LOOKING FOR:
    A Case Manager who will ensure that an individualized care plan and program is developed and implemented for patients and their family during their hospital stay and after discharge from the facility. Confirms safe and effective care prior to admission and through discharge for all patients.
    • Reviews medical records for potential admissions and assists with admission process. May include verifying insurance and patient assessment based on information provided, ensuring safe and effective care within our facility
    • Assessing clients' physical and mental wellness, needs, preferences and abilities to develop tailored care plans.
    • Working with clients, family and support networks and health care professionals to put care plans in place.
    • Recording clients' progress in EMR.
    • Evaluating clients' progress periodically and adjusting as needed to improve outcomes.
    • Maintain communication with pending patients, their families and referral sources and routinely follow the progress of prospective patients.
    • Conducts evals to determine the patient's discharge plan, discharge needs, and emotional/social needs.
    • Assists with arranging post-discharge services.
    • Participates in program development.
    • Participates in team and family conferences, as necessary.
    • Communicates with the clinical staff as needed for clarification of the patient's status and update them on the discharge plan.
    Education/Experience
    • Bachelor's or Master's Degree in Psychology or Human Services required.
    • 3-5 years of clinical experience, preferably in case management, or related experience.
    Licensure/Certification
    • Certified Case Manager (CCM) preferred.
