Job Summary:

Serves as the Clinical Manager responsible for supervising the activities of the home/health/hospice agency. Monitors quality, service, and utilization standards. Oversees the day-to-day clinical operational activities. Ensures compliance with regulations at all levels.



Essential Responsibilities:

  • Manages the day-to-day clinical operations, ensuring practice standards, staffing, payroll, budgets, fiscal management, and quality improvement.
  • Supervises agency and employee safety programs as well as risk management.
  • Responsible for the overall supervision of hospice and home health nurses and aides, including recruitment, hiring, task assignments, and personnel evaluations.
  • Develops and executes action plans to enhance staff development.
  • Acts as a valuable resource to the team as a clinical expert.
  • Ensures personalized care goals are achieved.
  • Guarantees the delivery of top-quality care in compliance with regulatory requirements and departmental policies.
  • Monitors service quality, utilization standards, and takes specific responsibility for patient care at the agency level.
  • Researches and implements best practice models from other agencies.
  • Participates in program management activities, including licensing, budgeting, and quality assessment/improvement.
  • Ensures well-coordinated treatment plans, customer-centered care, and cost-effective service utilization.
  • Monitors personnel allocation and utilization based on changing patient needs, ensuring optimal care levels and identifying potential cost-saving opportunities.
  • Collaborates with external healthcare providers to ensure optimal patient care.
  • Coordinates with team members or contract services for clinical care of hospice and home health patients and families.
  • Reviews all visits by contract services for adherence to policies, ensuring quality care through documentation reviews, staff visits, care plan assessments, multidisciplinary team conferences, and patient satisfaction evaluations.

Kaiser Permanente conducts periodic compensation reviews for positions. The organization reserves the right to reassess job descriptions or change positions from salaried to hourly pay status with prior notice to affected employees.



Basic Qualifications:

  • Five (5) years of nursing experience, with at least two (2) years as a Registered Nurse in a HH/H agency, primary care clinic, or health facility within the last five (5) years.

Education:

  • Graduate of an accredited nursing school.

License, Certification, Registration:

  • Registered Nurse License (California)
  • Driver's License (California)
  • Basic Life Support certification


Additional Requirements:

  • Demonstrated experience in supervision or leadership roles.
  • Knowledge of quality improvement practices, clinical care delivery processes, staffing, and budgeting.
  • Familiarity with Nursing Practice Act, The Joint Commission, and other regulatory requirements at federal, state, and local levels.
  • Understanding of standards and guidelines for Home Health/Hospice services.
  • Proficiency in ongoing monitoring techniques, care delivery in home environments, total quality management principles, and effective communication and leadership skills.
  • Ability to work in a Labor/Management Partnership setting.


Preferred Qualifications:

  • Public Health Nurse certificate preferred.
  • Prior experience in Home Health/Hospice preferred.
  • Bachelor's degree in nursing or a related healthcare field preferred.
